Enrolled at UF as a full-time undergraduate student (minimum of 12 credits hours) in the Fall following your Spring application (transfer students must have completed at least one semester at UF to be eligible to apply)
A minimum UF GPA of 3.0

Include a brief narrative with your application on how your life has been affected by a heart defect. Note:
First preference shall be for recipients who have a heart defect
Second preference shall be for recipients who have a family member or loved one impacted by a heart defect
Third preference shall be for recipients who are pre-med majors
Applying for Dean’s Office Undergraduate Scholarships
To apply for this scholarship, you must complete this online application and select up to three scholarships from the Dean’s Office Undergraduate Scholarship list, including this one. No more than one CLAS scholarship may be awarded to an applicant each year. We recommend reviewing the requirements and selecting the ones that you feel you are most qualified for. Late or incomplete applications will not be accepted.
As part of your application, you will submit an essay (no more than 500 words) on a topic to be announced in late January. You may also include separate documents listing your extracurricular activities, work experience, and honors and awards. (All documents must be in .pdf, .doc, or .docx format.)
